Quick Answer

Marionette lines are the vertical creases running from the mouth corners down toward the chin, often giving the face a downturned or aged appearance. At ID Cosmetic Clinic in Markham and Toronto, physician Dr. Dan Xu, M.D., CCFP personally treats marionette lines and downturned mouth corners using Health Canada–licensed dermal fillers and/or neuromodulators, which restore lost volume and relax the depressor muscles to soften creases and lift the corners of the mouth. Most patients see noticeable improvement after one to two sessions, with results typically lasting six to eighteen months depending on the product used.

WHAT ARE MARIONETTE LINES

What are marionette lines? Why are they more complex than a simple wrinkle?

Marionette lines — also called corner mouth lines or downward mouth folds — are two symmetrical grooves that run from the corners of the mouth down toward the jawline. The name comes from marionette puppets, whose mouths appear to be pulled downward by strings, mirroring how these lines look. TheyNot simply a surface skin wrinkle, but ratherVolume loss + skeletal support decline + muscle pull + skin laxitya composite aging sign resulting from four overlapping mechanisms.

Why can’t treatment be limited to injecting at the mouth corners?

the root cause of oral commissure descent lies inMid-face zygomatic arch volume loss + pre-jowl sulcus volume depletion。Injecting HA directly into the mouth corners only makes them look thicker and heavier, worsening the downturned appearance. Treatment must follow the MD Codes restoring volume from upstream to downstream。

Why do marionette lines appear as early as age 35 in Asian patients?

Asian faces tend to have a relatively flatter zygomatic arch and a shorter mandibular ramus, with less mid-face volume reserve. Once collagen is lost and the lower face loses structural support, marionette lines tend to develop more prominently than in Caucasian facesappearing 5–10 years earlier than expected。

Why do I look sad or stern?

A downturned mouth is one of the key signals the brain uses to perceive sadness, dissatisfaction, or aging. Even when you are perfectly content, these lines can cause others to instinctively read you as “unhappy” — this is the defining concern of marionette lines:social downtime。

Why is an MD Codes assessment required before treatment?

MD Codes was developed by Dr. Mauricio de Maiozonal facial aging assessment framework,By mapping 8 core facial zones to identify the true sites of volume loss, MD Codes avoids the trap of simply treating wherever the concern appears. Dr. Xu is an Allergan/Juvéderm North America Trainer and strictly follows the MD Codes protocol.

Clinical notes:Many clients come to us after visiting other clinics — they pointed to the corners of their mouth and asked to “fill right here,” and the provider did exactly that, injecting HA directly into the mouth corners. A few months later, the corners appeared thicker, the smile looked stiff, and jowling actually worsened. This is one of the most common missteps in Marionette lines treatmentThe most common misconception。Dr. Xu’s protocol: assess the upstream structures first, then address the lower face.

CAUSES

5 Root Causes of Marionette Lines — It’s Not Just About Aging

Clinically, marionette lines must be presentmultiple overlapping mechanismsfor deep grooves to develop. A single contributing factor produces only superficial lines — deep grooves are always the result of multiple overlapping causes.

1. Mid-face volume loss

As the suborbicularis oculi fat (SOOF) and buccal fat pad descend with age, the mid-face loses its natural lifting support, and lower-face soft tissue slides toward the mouth corners under gravity — this isthe single most important root cause。

2. Mandibular bone resorption / decline in skeletal support

40 With age, significant bone resorption occurs along the mandibular ramus and pre-jowl sulcus. The jawline loses its structural “platform,” and the overlying skin essentially hangs off the corners of the mouth, forming a groove. This is precisely why treating with hyaluronic acid filler alone addresses the symptom, not the underlying cause.

3. DAO overactive depressor anguli oris muscle

DAO(Depressor Anguli Oris)This is the muscle that directly pulls the corners of the mouth downward. Chronic negative facial expressions, masseter compensation, and poor dental occlusion can all cause the depressor anguli oris (DAO) to remain in a state of chronic tension, deepening the downturn. Micro-dose Botox® relaxation of the DAO is a critical step in treatment.

4. cutaneous collagen loss + reduced skin elasticity

30 After a certain age, collagen diminishes by approximately 1% per year. As skin loses its elasticity under the combined effects of gravity and muscle pull, lines become “set” at the surface level — meaning even filler may leave a faint residual crease. This is where Sculptra® or Sofwave™ becomes necessary to addresscollagen remodeling。

5. Dental health / occlusion / weight fluctuation

Missing teeth, dental misalignment, and rapid weight loss can all alter the distribution of soft tissue in the lower face. Clinically, many clients presenting with marionette lines actually need to address underlying dental issues or achieve weight stability before aesthetic filler treatment is appropriate.

DIFFERENTIAL DIAGNOSIS

4 lower-face concerns commonly confused with marionette lines — key points for differential diagnosis

Misclassification leads to treating the wrong area. The table below outlines the 4 most common differential presentations Dr. Xu encounters in clinical practice.

Nasolabial Folds

Nasolabial Folds · Nasolabial folds
LocationExtending from the nasal ala to the outer corners of the mouth
directiondiagonally downward and outward
contributing factorsPrimarily driven by mid-face volume loss
Relationship to the mouth cornersending just above the corner of the mouth
first-line treatmentZygomatic arch + nasolabial fold HA filler

marionette lines (primary condition)

Marionette Lines · Marionette lines
LocationDownward extension from the mouth corners to the jawline
directionVertically downward
contributing factorsMid-face + mandible + DAO complex
Relationship to the mouth cornersOriginating at the mouth corners and extending downward
first-line treatmentZygomatic arch + pre-jowl sulcus + depressor anguli oris (DAO) + mouth corners

smoker’s lines / perioral lines

Smoker’s Lines · Lip Lines
Locationradial fine lines around the upper and lower lips
directionvertical lip border
contributing factorsrepeated orbicularis oris muscle contraction + photoaging
Relationship to the mouth cornersdoes not extend to the jawline
first-line treatmentmicro-dose perioral Botox® + superficial HA / picosecond laser

Jowls

Jowls · Double chin / jowling
LocationLateral jawline protrusion
directionSagging along the jawline
contributing factorsSMAS Fascial laxity + bone resorption
Relationship to the mouth cornersCommonly appears alongside corner mouth droop
first-line treatmentSofwave / Ultherapy + Radiesse jawline contouring

⚠️ Important:Clinically, approximately 40% of clients who present complaining of nasolabial folds actually have marionette lines — and vice versa. The two are anatomically adjacent and mechanistically related, yet the injection points are entirely different. Misdiagnosis leads to treating the wrong area, resulting in limited improvement and an unnatural appearance — this is the most common reason lower-face filler treatments fail.

ASIAN SKIN FOCUS

Why do marionette lines in Asian faces require a dedicated Asian lower-face protocol?

A common blind spot in mainstream North American clinics

Most lower-face filler protocols used in North America are derived from anatomical data based on Caucasian features — prominent cheekbones, a longer jawline and thicker skin. Applying these protocols directly to Asian faces creates three core problems:

  • ✗ Overfilling along the zygomatic arch —— Asian patients naturally have a flatter zygomatic arch — overfilling in this area can create a widened or prominent cheekbone appearance
  • ✗ Excessive dosing in the pre-jowl sulcus —— The Asian mandibular ramus tends to be shorter, making the lower face more prone to appearing full or square
  • ✗ Directly piling HA filler at the mouth corners —— Overlooking the DAO muscle factor, resulting in a stiff, heavy lower face

Dr. Xu’s specialized Asian lower-face refinement

parametersNorth American standardsAsian-specific adjustment
Total zygomatic arch filler volume2–4 mL0.8–1.5 mL
Pre-jowl sulcus dosage1–2 mL / side0.3–0.8 mL / side
Direct mouth-corner filler injectionCommonly performedrare; depressor anguli oris Botox® is the preferred first-line option
Injection depth / planemultiple superficial layersSupraperiosteal + deep fat compartment
Staged pacing1 sessions to complete2–3 staged assessments

Key differentiating factors:With 8 years of dedicated practice treating Asian faces at our Markham/Toronto clinic, and as an Allergan/Juvéderm North America Official Trainer, Dr. Xu has systematically integrated the MD Codes framework with the anatomical nuances specific to Asian lower-face structure. Our protocol is designed to achieveNatural transition + subtle lift + no artificial appearance, rather than the Caucasian aesthetic of “volume + definition.”

TREATMENT OPTIONS

6 Treatment Options for Marionette Lines — Tailored by Severity and Combined as Needed

There is no single “miracle treatment” for downturned mouth corners. We combine injectable fillers and energy-based devices based on MD Codes assessment and severity.

Recommended

Radiesse(calcium hydroxylapatite)

A structural, bone-level filler that provides immediate volume while stimulating long-term collagen production — particularly well suited forjawline contouring + significant bone resorption。Dr. Xu uses this for jawline recontouring, not for direct mouth-corner injection.

Learn about Radiesse® →

Adjunct

Botox Micro-dose DAO injection

2–3 units injected precisely into the depressor anguli oris on each side, releasing the downward pull. This is the “hidden key” to understanding marionette lines —— Without this step, filler results alone will be significantly compromised. Maintain every 4 months.

Adjunct

Sofwave / thread lift / skin tightening

Sofwave Ultrasound skin tightening to address SMAS laxity — well suited forReduced skin elasticity + overall lower-face ptosis。Thread lifting may serve as a short-term lifting option in carefully selected patients, but Dr. Xu does not recommend it as a primary treatment.

TREATMENT FLOW

Dr. Xu’s 6-Step Standard Protocol for Marionette Lines

01

consultation + MD Codes assessment

30 Free consultation, MD Codes 8-zone assessment, static and dynamic expression analysis, VISIA quantification of lower-face symmetry. No injection without assessment。

02

Upstream first — zygomatic arch / mid-face support

The first session prioritizes the zygomatic arch (Ck1/Ck2) and mid-face volume restoration to rebuild upward lift.no direct injection at the oral commissure。2 weeks later to assess the degree of improvement.

03

Pre-jowl sulcus + jawline contouring

Addressing Jw1 pre-jowl sulcus volume loss, and Jw2/Jw3 for jawline skeletal support reconstruction. This step is typically completed 2–4 weeks after the first treatment session.

04

DAO Botox + Mouth corner refinement

After relaxing the depressor anguli oris muscle, we reassess whether localized HA refinement at the oral commissure is needed at all. Many patients find that after steps 02–03, the corners of the mouth have already lifted naturally — no direct filler is required.

05

Collagen remodeling (moderate-to-severe cases)

Initiate a course of 3 Sculptra® treatments (once every 6 weeks) or Sofwave™ skin tightening, addressing skin elasticity at the structural level. This forms the foundation for long-term stability.

06

Maintenance phase (assessment every 12–18 months)

HA Filler maintenance every 12 months, Botox® every 4 months, Sculptra® every 18–24 months.regular micro-adjustments consistently outperform a single over-treatment。

「The most common reason marionette line treatments fail is not the product — it isInjector simply treating where the patient points。”When a patient points to their mouth corners and asks me to inject there directly, the result is thicker-looking corners and a stiff smile. My protocol is always: assess upstream first, then decide whether to touch downstream.” — Dr. Dan Xu, M.D., CCFP

Clinical note:The biggest mistake in treating marionette lines is injecting too much in a single session. Conservative dosing combined with staged reassessments consistently delivers more natural-looking results, longer-lasting outcomes, and fewer complications over time. Among Dr. Xu’s lower-face clients in Toronto and Markham, the follow-up satisfaction rate for those on a staged protocol exceeds 95%.

「In Markham — one of the most concentrated markets for aesthetic medicine in the Chinese-Canadian community — I have seen far too many patients who had filler stacked directly at the corners of their mouth, resulting in a thickened, stiff smile that actually made them look older. The issue was never the quality or cost of the product; it wasIs anyone willing to spend 30 minutes on a thorough assessment。」 —— Dr. Dan Xu

PRICING PHILOSOPHY

About the Cost of Marionette Line Treatment

Pricing for marionette line treatment varies widely from clinic to clinic — but the true cost is not measured in millilitres of filler. It reflects who performs the assessment, who determines the treatment sequence, and how deeply clinical expertise is required. Marionette lines involve multiple contributing factors: mid-face volume loss, skeletal support, muscle pull, and skin laxity. What ultimately determines the outcome is the quality of the overall treatment plan, not any single product. Complex cases that require MD Codes systematic assessment, dynamic and static analysis, and staged refinement involve Dr. Xu’s direct, in-depth participation — and the pricing reflects that level of medical resource commitment. For early-stage, mild cases,For clients: our trained medical team can safely carry out certain maintenance steps under a physician-directed protocol. All injectable treatments are performed by Dr. Xu personally — a standard rooted in both safety and aesthetic judgment. The gap between an injector and a physician shows up most clearly in decisions like “upstream-first” prioritization, staged reassessment, and DAO factor identification. Which approach is right for you is determined by your lower-face aging pattern, not by price — the specifics will be discussed during your consultation using MD Codes assessment.

RISKS & INFORMED CONSENT

Risks & Contraindications — What You Need to Know

What are the main risks?

1. Bruising / swelling(common, resolves in 3–7 days) — significantly reduced with fine-needle and blunt cannula technique; 2. Nodules at injection sites (rare; can be resolved with massage or dissolving agent); 3. Vascular occlusion(extremely rare but serious) — requires thorough knowledge of lower-face vascular anatomy; Dr. Xu employs a triple-safety protocol of aspiration, blunt cannula, and slow injection. 4. Overfilling leading to a “sausage lip” appearance or a heavier-looking chin (avoidable with a well-designed protocol). 5. Results not lasting (the natural half-life of HA is a reality; regular maintenance is needed).

Who should absolutely not have this treatment?

Pregnancy, breastfeeding, active autoimmune conditions, active skin infection or cold sore outbreak in the treatment area, allergy to hyaluronic acid or lidocaine, recent dental surgery (within 2 weeks), severe keloid tendency that has not been assessed, and unrealistic expectations (believing one session will permanently resolve the concern) — in any of these situations, Dr. Xu will recommend postponing treatment or addressing the underlying issue first.

How are complication risks minimized during treatment?

5 Core Principles: (1) MD Codes assessment and anatomical mapping are mandatory before every session; (2) blunt cannulas are preferred, with slow aspiration in high-risk zones; (3) conservative dosing per session — results are built gradually across multiple visits; (4) treat upstream structures first — never stack volume directly at the corner of the mouth; (5) for 48 hours post-treatment, avoid heat, intense exercise and alcohol — return immediately if you experience persistent pain or blanching.

What does the informed consent form include?

Every client is required to sign an Informed Consent form prior to their first treatment. This covers: product ingredients, injection mechanism, possible adverse reactions and their likelihood, vascular emergency protocol (hyaluronidase is always on hand at the clinic), alternative options, follow-up arrangements, and a Media Release (optional). All content is explained by Dr. Xu personally — not delegated to administrative staff.

TORONTO PATIENTS ASK

Toronto Marionette Lines Treatment FAQ

Answered personally by Dr. Dan Xu — based on the most frequently asked questions from lower-face rejuvenation clients in Toronto and Markham.
Does hyaluronic acid filler actually work for marionette lines? Can repeated treatments make the drooping worse?

effective — but only when placed in the right anatomical location.the root cause of oral commissure descent lies inmid-face support collapse + volume loss at the pre-jowl sulcus, and not the oral commissure itself. Dr. Xu's protocol follows the MD Codes upstream-first:By first restoring support at the zygomatic arch and pre-jowl sulcus, the oral commissure lifts naturally. Placing filler directly at the corner of the mouth alone thickens the area and worsens jowling — this is the "the more you inject, the more it droops" trap that catches so many patients.

Are marionette lines and nasolabial folds the same thing?

No.Nasolabial foldsis the groove that extends diagonally downward and outward from the alar base to the lateral aspect of the oral commissure, ending just above the corner of the mouth;Marionette linesare grooves that run vertically downward from the corners of the mouth to the jawline. The two conditions share overlapping mechanisms but require entirely different injection points. Clinically, approximately 40% of clients who present complaining of "nasolabial folds" actually have marionette lines, and vice versa — an MD Codes assessment is essential to differentiate them.

Why is Dr. Xu recommending Botox® for me? I don't have any movement wrinkles there.

becauseDAO Depressor anguli oris (DAO)is the muscle that directly pulls the mouth corners downward; chronic tension in this muscle deepens the appearance of corner droop. Dr. Xu relaxes the DAO with a micro-dose (2–3 U per side), allowing the mouth corners to return naturally to a neutral position. This step isThe Hidden Key to Marionette Line Treatment —— Without this step, filler results alone will be significantly compromised. The Botox® dose used is minimal and will not affect your smile.

Juvéderm or Restylane — which is better?

Each has its own strengths:Juvéderm Voluma Provides strong structural support, ideal for deep supraperiosteal injection along the zygomatic arch;Restylane Defyne with excellent flexibility and natural movement — well suited to the high-mobility area around the oral commissure;Juvéderm Volbella Ideal for fine refinement around the corners of the mouth. As an Allergan/Juvéderm North America Trainer and holder of advanced Galderma Restylane certification, Dr. Xu selects the most appropriate product for each specific area — rather than defaulting to a single filler for everything.

Sculptra Is Sculptra® right for me? How does it differ from hyaluronic acid filler?

Hyaluronic acid (HA) isImmediate volumization(results visible on the day of injection), Sculptra® isCollagen stimulator(3 months of progressive improvement). If you areModerate to severe downturned mouth corners + generalized lower-face laxity,Dr. Xu typically recommends a course of 3 Sculptra® treatments for structural collagen rebuilding, paired with targeted hyaluronic acid filler to address finer groove lines. Sculptra® results last 18–24 months, with a gradual, natural progression — no sudden change in appearance — though proper post-treatment massage is essential.

Can I have marionette line treatment while pregnant or breastfeeding?

No.All injectable treatments (HA fillers / Botox® / Sculptra®) are relatively contraindicated during pregnancy and breastfeeding. Facial volume fluctuates significantly during this period, making it difficult to assess treatment outcomes reliably. We recommendpostpartum and at least 3 months after fully stopping breastfeedingbefore beginning the assessment. In the meantime, consistent sleep, a regular skincare routine, and diligent sun protection can help slow the aging process.

How many sessions does oral commissure descent treatment require? When can I expect to see results?

Depends on severity:early-stage: 1–2 sessions(Results visible immediately after injection, stabilizing over 2 weeks),Moderate: 2–3 staged sessions(2 months), with notable improvement withinsevere cases need 3–4 sessions + Sculptra® treatment(4–6 months of progressive improvement). All patients are enrolled inMaintenance cycle every 12–18 months. Dr. Xu insists on staged assessments to avoid overtreatment in a single session.

I had filler done elsewhere and my mouth corners look thicker and heavier now. Can this be corrected?

Yes. This is a classic case of localized overfilling caused by "injecting where the patient points." Dr. Xu's revision approach: (1) staged hyaluronidase dissolution of excess HA (2 sessions, 4 weeks apart); (2) wait 4 weeks for tissue repositioning; (3) re-inject following the MD Codes upstream-firstprotocol. The full revision cycle takes approximately 3–6 months and can restore a natural, smooth transition.

How soon can I return to normal activities after treatment for oral commissure descent? Will anyone be able to tell?

HA / Botox InjectionNo significant downtime —— You can resume your normal routine on the day of treatment. Possible reactions include: (1) small pinpoint redness at injection sites (resolves within a few hours); (2) mild swelling (24–48 hours); (3) bruising in a small minority of patients (3–7 days). Dr. Xu's "upstream-first + regular micro-adjustment" philosophy is designed to achieveGradual, natural progression,Others will simply think you look refreshed lately — not that you've had anything done.

I live in downtown Toronto / North York / Richmond Hill / Scarborough — is it worth making the trip to Markham?

ID Cosmetic Clinic Located at 30 Gibson Dr., Markham — 30–40 minutes by car from downtown Toronto, 20–25 minutes from North York, 15–20 minutes from Richmond Hill / Vaughan, and 20–30 minutes from Scarborough. Conveniently situated at the intersection of Highway 7 and Highways 404 / 407.Marionette line treatment is a long-term commitment,This treatment requires ongoing physician follow-up and fine-tuning — choosing the right physician matters far more than proximity. The cost of correcting an over-treated result far exceeds the inconvenience of a single commute.
