Quick Answer
Axillary hyperhidrosis is a medical condition characterized by excessive underarm sweating beyond what the body needs for temperature regulation. It affects people of any age or skin type whose sweating significantly disrupts daily life or clothing choices. Treatment uses targeted injections of botulinum toxin to temporarily block the nerve signals that activate sweat glands. At ID Cosmetic Clinic in Markham and Toronto, this treatment is personally performed by physician Dr. Dan Xu, M.D., CCFP. Most patients see results within one to two weeks, with effects typically lasting four to six months per session.

What Is Axillary Hyperhidrosis?
Axillary HyperhidrosisRefers to localized axillary sweat production that exceedsRequired for physiological thermoregulation, is Primary focal hyperhidrosis (PFH)The most common type. Affecting approximately 2–3% of the population, its impact is significant yet widely underestimated — most patients are unaware that this is a medical condition that can be treated or controlled.
5 Key Differences Between Primary and Secondary Hyperhidrosis
Age of onset — typically around puberty
Approximately 90% of primary hyperhidrosis cases develop before or during puberty, with patients often feeling”You sweat excessively”. Secondary hyperhidrosis typically appears suddenly in adulthood and warrants investigation for systemic disease.
Symmetric distribution — equal on both sides
Typical presentation of primary hyperhidrosisBilateral symmetry(Both underarms, both hands, both feet, etc. Unilateral or generalized hyperhidrosis requires investigation for neurological, endocrine, or medication-related causes.
Stops at night — does not affect sleep
The hallmark features of primary hyperhidrosis are“Stops at night” — No sweating occurs during sleep. If night sweats are significant, secondary causes such as infection, malignancy, or hormonal imbalance should be investigated.
Positive family history — 30–50% have a familial predisposition
Approximately half of patients have a positive family history (a parent or sibling with the same condition), suggestingAutosomal dominant inheritance. However, the absence of a family history does not rule out primary hyperhidrosis.
Triggers — emotions, temperature, and food can all worsen the condition
Primary hyperhidrosisStress, heat, caffeine, spicy foodsignificantly worsens under these triggers, affecting both social interactions and work performance. Following Botox® treatment, the impact of these triggers is markedly reduced.
Severity Assessment (HDSS Scale)
- Grade 1:Sweating is barely noticeable and does not affect daily activities
- Grade 2:Sweating is noticeable and occasionally interferes with daily activities
- Grade 3:Sweating is almost constantly visible and frequently interferes with daily activities
- Grade 4:Sweating is severe and consistently interferes with daily activities
HDSS 3 Grade and above is consideredClinically significant hyperhidrosis,Botox Treatment has clear indications.
3 Major common lifestyle impacts
Clothing stains
White or light-colored shirts are perpetually stained under the arms, even with sweat pads. Many patients have worn only dark-colored clothing for years as a result.
Social anxiety
Afraid to raise your arms, afraid to hug, afraid to interview. Sweating → anxiety → more sweating: a vicious cycle.
Professional impact
Professions such as sales, consulting, public speaking, and performing are significantly affected. Important meetings, weddings, and major occasions frequently become sources of anxiety.
5 Step-by-step self-assessment
- Are you currentlyWhen not hot or stressedStill sweating continuously?
- Whether sweatingBilateral symmetry?
- Nighttime sleepDoes it stop sweating entirely?
- Whether you areAround pubertydoes sweating begin?
- Whether it affectsClothing choices, social life, work?
3 or more items”yes”,You may be a strong candidate for Botox treatment of primary axillary hyperhidrosis. During your consultation, Dr. Xu will perform the Minor starch-iodine test for precise sweat zone mapping.
Causes and Mechanisms of Axillary Hyperhidrosis
The underlying mechanism of primary axillary hyperhidrosis isExcessive sympathetic cholinergic signalingEccrine glands — not an abnormal number of sweat glands — are the source of the problem. Understanding this is key to understanding why Botox® is the most effective treatment.
Genetic factors
30–50% Patients frequently report a positive family history, suggesting an autosomal dominant inheritance pattern. The specific genes have not yet been fully identified, but the condition is associated with dysregulation of sympathetic nervous system activity.
Sympathetic overactivation
Under normal conditions, sweating is regulated by the hypothalamic thermoregulatory center via cholinergic sympathetic nerve fibers that control the sweat glands. In patients with primary hyperhidrosis,This regulatory system overreacts to stimuli.
Emotional triggers (sympathetic activation)
Stress, anxiety, and social situations activate the sympathetic nervous system → increased sweating → heightened anxiety → a vicious cycle. This is why many patients”The more anxious, the more you sweat”.
Temperature and food triggers
Hot environments, hot beverages, spicy food, caffeine, and alcohol can all worsen sweating. These are everyday triggers that are difficult to avoid entirely.
Secondary causes (conditions to rule out)
If sweating isSudden onset, unilateral, generalized, prominent at night, accompanied by weight loss,Underlying causes to rule out include: hyperthyroidism, diabetes, lymphoma, infection, pheochromocytoma, medications (antidepressants, hormones), and menopause, among others.
Primary hyperhidrosis vs. secondary / anxiety-related / bromhidrosis
Not all”Excessive sweating”Not everyone is an immediate candidate for Botox® — it is essential to confirm whether the condition is primary and whether any co-existing issues are present before proceeding.

Treatment considerations for axillary hyperhidrosis in Asian patients
Treatment of axillary hyperhidrosis in Asian patients differs slightly from Western populations in terms of dosage, social context, and co-existing concerns — however, the overall treatment protocol remains consistent.
1. A combination of axillary bromhidrosis and hyperhidrosis is common
Asian patients, particularly those of Chinese heritage, tend to have less active apocrine glands compared to European, North American, or Middle Eastern populations; however, a significant proportion of patientsat the same timeExcessive underarm sweating (eccrine gland overactivity) and underarm odor (apocrine glands) are distinct concerns. Botox® primarily treats hyperhidrosis; its effect on odor is limited.
2. High professional appearance demands
In Asian urban professional environments — particularly finance, consulting, sales, and healthcare —Frequently wearing formal attire,Stained white shirts and ruined silk blouses are all too familiar. Excessive underarm sweating can significantly impact your professional image — making effective treatment a pressing priority.
3. Weddings / important occasions
Asian weddings typicallyWhite wedding dress + extended toasting,Any underarm dampness is”Wedding nightmare”. Dr. Xu has extensive clinical experience with preventive treatments scheduled 6 weeks before weddings.
4. Personalized dosage
Asian patients tend to have a smaller overall body frame, but dosing for axillary hyperhidrosis is primarily determined byUnderarm zoneDosage is determined by clinical assessment, not body weight. Dr. Xu uses Minor’s test to precisely map the treatment area and tailor the dose, typically 50–100 units per side.
Dosage reference × severity
| HDSS | Sweating area | Dosage per side |
|---|---|---|
| Grade 2 | < 5×5cm | 30–40 units |
| Grade 3 | 5–8cm | 40–50 units |
| Grade 3–4 | 8–12cm | 50–75 units |
| Grade 4 | > 12cm | 75–100 units |
Axillary hyperhidrosis is one of the Botox® clinical applications in which“Most rewarding”one of the procedures — It delivers direct, meaningful, and reproducible improvements to patients’ quality of life. Many patients tell me after treatment:””If I had known it was this straightforward, I never should have suffered through 10 years of this.”” As an Allergan/Juvéderm North America Trainer, axillary hyperhidrosis with Botox® is one of the standard cases I teach to other physicians. — Dr. Dan Xu, M.D., CCFP
Maintenance frequency reference
- Most patients return every 5–7 months
- Severe cases may last 4–5 months
- Mild cases and cooler seasons may extend results to 9 months
- For best results, treat 4–6 weeks before summer
Axillary Hyperhidrosis Treatment Options (Botox® is the Gold Standard)
FDA Botox® (onabotulinumtoxinA) has been approved for axillary hyperhidrosis for over 20 years, with a clinical efficacy rate exceeding 90%, making itMost effective, safest, and longest-lastingnon-surgical treatment.
Botox® Underarm injection Gold standard
Health Canada–licensed for axillary hyperhidrosis since 2004, with a clinical efficacy rate of 90%+. As an Allergan/Juvéderm North America Trainer, Dr. Xu’s expertise in Botox® injections for axillary hyperhidrosisPrecise grid mapping + personalized dosageis one of his signature procedures.
- 50–100 units per side (based on treatment area)
- 1–1.5cm Grid pattern · 25–50 multi-point injections
- 4–7 Days to onset · full effect at 2 weeks
- Results last 5–9 months
- Minimal downtime · Return to work the same day
Minor Starch-iodine test Essential for diagnosis
The gold standard for diagnosis and mapping: iodine solution is applied to the underarm, allowed to dry, then starch is dusted over — active sweat zones turn dark purple-black, precisely identifying the treatment area and guiding Botox injection point design.30 Minutes to complete · safe and comfortable.
Topical anesthesia + ice application Comfort optimization
Topical lidocaine applied 20 minutes before injection, combined with ice packing, significantly reduces discomfort during multi-point injections. Both are standard components of Dr. Xu’s protocol to ensure a comfortable treatment experience.
Prescription antiperspirant First-line option for mild cases
Prescription antiperspirants containing aluminum chloride (20% or higher), such as Drysol, aremildA first-line treatment for hyperhidrosis that can be combined with Botox® to help maintain results. Note: nighttime skin irritation may occur.
Oral anticholinergic medications Alternative options
Oral medications such as oxybutynin may provide short-term relief, butNoticeable side effects(dry mouth, constipation, and drowsiness — making it unsuitable for long-term use. Botox® is a more reliable option.
miraDry® / Microwave ablation Permanent option
Health Canada–licensed microwave technology,Permanent destruction of sweat glands. Advantages: permanent results. Disadvantages: lower efficacy than Botox® (75% vs. 90%+), prolonged post-procedure swelling, higher cost, and suboptimal outcomes in some patients. We do not offer miraDry in-clinic, but clients are welcome to ask us to help compare options.
Endoscopic thoracic sympathectomy (ETS) Extreme option
Surgical procedure: thoracoscopic resection of the sympathetic nerve chain. Not recommended for axillary sweating alone — High risk, with compensatory sweating occurring in 60%+ of cases (increased sweating elsewhere on the body). Reserved only for severe cases where all other options have failed.
Treatment Decision Chart
| Severity | Recommended treatment plan |
|---|---|
| HDSS 1–2 mild | Prescription antiperspirant → if no relief, proceed to Botox |
| HDSS 3 Moderate | Botox First-line choice |
| HDSS 4 Severe | High-dose Botox® + long-term maintenance |
| Botox Repeatedly effective but seeking a permanent solution | Discuss miraDry options |

About the Cost of Botox® Treatment for Axillary Hyperhidrosis
Many patients ask during their first consultation:”How many units are needed and what is the cost?”However, this type of”Priced per unit”The way questions are framed can lead you to decisions that are not in your best interest. When it comes to Botox® for axillary hyperhidrosis,90% of treatment effectiveness depends on whether the dosage is sufficient to fully cover your sweating zone — If the goal is”Save units”Under-dosing leads to suboptimal results — and that is simply a waste. That is why we always perform a Minor starch-iodine test first, mapping your actual sweat distribution to determine the right individualized dose for you.
At ID Cosmetic Clinic, axillary hyperhidrosis with Botox® Performed personally by the physician — Because treatment involves a multi-point grid design, individualized dosage decisions, and clinical judgment around touch-up injections — all of which directly affect outcomes and longevity — physician oversight is essential. As an Allergan/Juvéderm North America Trainer, Dr. Xu uses axillary hyperhidrosis injections as a standard case in her physician training program. Routine maintenance visits for established long-term patients with a stable protocol may be performed by trained clinical staff under physician supervision, and are priced accordingly.
We do not publish a standard price list online, because doing so shifts focus to numbers rather than the factors that truly determine outcomes — your sweat zone, HDSS severity, target dryness level, and desired maintenance frequency all shape the right plan and its cost. After a Free 30-Minute Consultation, Dr. Xu will clearly outline your sweat zone, recommended dosage, and corresponding fee structure so you can make a fully informed decision.Treatment decisions should be driven by clinical need, not price.

Toronto Axillary Hyperhidrosis — FAQ
What Is Axillary Hyperhidrosis? How Does Treatment Work?
Axillary hyperhidrosis (excessive underarm sweating) is a physician-led program offered at ID Cosmetic Clinic in Markham/Toronto. All consultations and treatment plans are conducted personally by Dr. Dan Xu, M.D., CCFP, Allergan/Juvéderm North America Trainer. Because mechanisms, devices, and indications vary by individual, we recommend a Free 30-Minute Consultation for a personalized assessment.
Who Is a Good Candidate for Axillary Hyperhidrosis Treatment?
Most healthy adults (18+) may be considered candidates following a consultation and assessment. Not suitable for:Pregnancy / breastfeeding / active flare of a serious chronic condition / active infection at the injection site / known severe allergy history. Your specific candidacy will be assessed by Dr. Xu during your consultation, based on your medical history, anatomy, and treatment goals.
How long do results for axillary hyperhidrosis last?
Results depend on your specific treatment plan, individual metabolism, and maintenance frequency. During your consultation, Dr. Xu will provide clear, personalized expectations — We do not promise outcomes beyond what is medically reasonable。
What Are the Side Effects and Risks of Axillary Hyperhidrosis Treatment?
All aesthetic procedures carry inherent risks; however, oversight by Dr. Dan Xu, M.D., CCFP, combined with strict sterile technique and personalized dosage and depth assessment, keeps those risks within a reasonable, manageable range. Common mild reactions such as bruising and swelling typically resolve within 5–10 days. A detailed informed consent form will be reviewed and signed following your consultation.
Can underarm neurotoxin injections permanently cure hyperhidrosis?
Cannot be permanently cured, but can be effectively managed long-term。Botulinum toxin blocks acetylcholine release at nerve terminals. Results gradually wear off after 6–8 months, requiring periodic maintenance treatments. However, clinical observation shows thatClients who have received regular injections for 2–3 or more years generally experience a reduction in baseline perspiration,Even after stopping treatment, baseline sweat levels remain lower than before. This reflects a chronic condition management approach — not a one-time cure.
Is the underarm botulinum toxin injection painful? Is anesthesia required?
The underarm skin has sparse nerve distribution, making it less sensitive than facial injection sites. Dr. Xu uses 30G ultra-fine needles combined with a 10-minute ice pack application before treatment. The vast majority of clients describe the sensation as a mild pinch that is easily tolerated.EMLA topical anesthetic cream is not recommended,as it can temporarily affect sweat gland assessment and Minor's test results. The full bilateral underarm injection takes approximately 15–20 minutes.
Botox、Dysport、Xeomin What is the difference when used for hyperhidrosis?
All three are botulinum toxin products, but they differ slightly in molecular structure and onset time:Botox(onset in 3–7 days, results lasting 6–8 months, with the most extensive published evidence;DysportFaster onset of 2–4 days, slightly greater diffusion, suitable for severe or large-area hyperhidrosis;Xeomin(Pure-form neurotoxin) contains no accessory proteins, making it suitable for clients who have developed antibodies to Botox® or experienced diminishing response. Dr. Xu will recommend based on your individual response and medical history.
Why do some people see no results from neurotoxin injections?
Three common reasons: (1) Minor's test not performed,Grid points placed outside the actual sweating zone, leading to missed coverage; (2) Insufficient dosage—— price-driven clinics cut costs by using only 50–60 units total for both underarms (effective dosing typically requires 80–120 U); (3) Antibody formation—— After multiple injections, neutralizing antibodies may develop; switching to a protein-free formulation such as Xeomin® can restore effectiveness.
How long do results last?
Clinical average:Women: 6.5–7.5 months; Men: 5–6 months(faster metabolism). Those with severe hyperhidrosis tend to have shorter results than those with mild cases. Duration is also affected by activity level, metabolism, and individual physiology. Dr. Xu recommendsTouch-up 4 weeks before summerCovering the full peak sweating season for optimal results.
Is it safe to have injections during pregnancy or breastfeeding?
No.The safety of neurotoxin injections during pregnancy and breastfeeding has not been sufficiently established by clinical evidence. Both the FDA and Health Canada classify it as a Category C risk. We recommendPostpartum + at least 3 months after fully stopping breastfeedingwaiting until this stage has passed before beginning an assessment. For those with pressing needs during this period, prescription-strength aluminum chloride antiperspirant (Drysol) may serve as a temporary bridge — Dr. Xu will assess whether it is appropriate for you.
Can it cause compensatory sweating (increased perspiration in other parts of the body)?
Clinical literature reports the incidence of compensatory sweating following axillary neurotoxin injection as <5%,and typically mild(Any mild increase in perspiration on the back or chest is generally not bothersome to patients. This is entirely different from the severe compensatory sweating seen in 50–80% of patients following endoscopic thoracic sympathectomy (ETS) surgery. If you have heard that "treating the underarms causes sweating all over the body," that is a surgical risk — not a risk associated with neurotoxin injections.
Minor What is the Minor's iodine-starch test, and why does Dr. Xu insist on performing it?
Minor The test is a classic dermatological procedure developed in 1928 — iodine solution is applied to the skin in a dim room, followed by starch powder; areas of active perspiration will revealDeep purple patches, precisely mapping the active sweat gland boundaries. This step determines the injection grid and dose distribution.Skipping the Minor's test and estimating the treatment area by eye increases the risk of missed sweating zones or wasted dosage—— This is the non-negotiable standard in Dr. Xu's protocol — and the step most commonly skipped at general medspas.
Can I work and exercise normally on the day of treatment?
Post-injectionMinimal downtime — return to normal activities the same day。That said, we recommend: (1) keep your upper body upright for 4 hours post-injection; (2) avoid strenuous exercise, rubbing the underarms, or using a sauna for 24 hours; (3) no baths (showers are fine); (4) no alcohol. You can return to all normal activities the next day, and the treatment takes effect within 7 days.
I live in downtown Toronto / North York / Richmond Hill / Scarborough — is it worth making the trip to Markham?
