Benefits of Professional Freckle Removal
PicoSure Pro® 755nm Targeted picosecond freckle removal · Results in 1–2 sessions · Minimal downtime · Safe for Fitzpatrick III–V
Quick Answer
Freckles are small, flat spots of concentrated melanin that commonly appear on sun-exposed skin, particularly in fair-skinned individuals with a genetic predisposition. At ID Cosmetic Clinic in Markham and Toronto, Dr. Dan Xu, M.D., CCFP, personally performs freckle treatments using laser and light-based technologies — such as IPL or Q-switched lasers — which target melanin to gradually fade pigmentation. Most clients see noticeable improvement over 2–4 sessions, spaced several weeks apart.

What Are Freckles? How Are They Different from Melasma and Sun Spots?
Freckles (Medical Term: Ephelides)is a condition caused byGenetic predisposition + UV-triggeredsmall, discrete pigmented spots, typically < 5mm in diameter, symmetrically distributed acrossBridge of the nose, upper third of both cheeks, and forehead. Most clients notice freckles as early as childhood (ages 5–10). They darken noticeably with increased UV exposure in summer and fade during winter.
5 Key Differences Between Freckles and Common Pigmented Spots
Primarily genetic — not simply “caused by sun exposure”
The primary cause of freckles isMC1R Genetic predisposition, UV exposure is an aggravating factor, not the root cause. This is why clients with a family history of freckles may develop them even with diligent sun protection — whereas sunspots (solar lentigines) are caused entirely by cumulative UV damage.
Appear in childhood — not in middle age
Freckles typically first appear between ages 5 and 10 and often increase during puberty. Melasma, by contrast, develops between ages 30 and 50 and is driven by a combination of hormonal changes and UV exposure.acquiredpigmentation condition.
Well-defined borders — small discrete spots that do not merge
Typical freckles measure 1–4mm in diameter, appearing as small discrete spots,Well-defined borders, non-confluent. Melasma, by contrast, presents as large, diffuse patches with poorly defined borders, distributed symmetrically over the cheekbones.
PIH risk is very low — high energy settings are well tolerated
Epidermal freckles have a straightforward structure. After targeted picosecond laser treatment,PIH Very low rate of post-treatment darkening (PIH). This is the exact opposite of melasma, which requires ultra-low energy settings and a much more cautious approach.
1–2 Results visible after one session — ongoing maintenance required
PicoSure Pro 755nm Clearance rates for freckles are very high,1–2 Significant fading or clearance achievable in one session. Ongoing care simply requires SPF 50+ sun protection and a once-yearly check-in at the end of summer. This is entirely different from the lifelong management required for melasma. For sessions, cost and what to expect, see our laser freckle removal page.
What Do Freckles Look Like? Typical Appearance
- shape: Flat, light- to dark-brown spots (1–4 mm in diameter; not raised, not painful, not itchy)
- Borders: TypicallyClear (unlike the diffuse, poorly defined borders of melasma)
- Symmetry: bilaterally distributed, not necessarily symmetrical (unlike the symmetrical, patch-like appearance of melasma)
- Color: Light to dark brown; more visible in lighter skin tones — freckles are actually less common in clients with deeper skin tones
- Seasonal: Darkens in summer, fades in winter (extremely high UV sensitivity)
3 common freckle distribution patterns
Nasal-cheek type (most common)
Distributed across the nose bridge and upper cheeks in a butterfly-like lateral spread. Approximately 70% of freckle clients present primarily with this pattern.
diffuse type
Distributed across the full face — including the forehead, chin, and temples. Most common in clients with lighter skin tones (Fitzpatrick I–II) and Northern European heritage.
Sun-exposed type
Distributed across chronically UV-exposed areas such as the neck, shoulders, chest, and back of the hands. Most common in outdoor workers and clients with a long history of inadequate sun protection.
4 step self-assessment
- Whether it isSmall spots (< 5mm) with well-defined borders?
- Whetherappearing as early as childhood (ages 5–10)?
- distributed onNose bridge / both cheeks / forehead?
- whether in summerNoticeably darker?
⚠️ If 3–4 criteria are met,Most likely freckles (ephelides), A consultation is recommended for definitive diagnosis. However, accurate diagnosis requires PicoSure Pro Pre-treatment Wood’s lamp examination — the treatment protocols for freckles, sunspots, melasma, and moles are entirely different from one another.
Common Causes of Freckles (5 Key Factors)
The key characteristic of freckles isGenetics > UV exposure > other factors. Understanding the weight of each triggering factor determines your treatment strategy and long-term sun protection plan.
Genetic predisposition (MC1R gene · strongest contributing factor)
Most patients with freckles carry MC1R gene variants, which cause melanocytes to over-respond to UV exposure. More than 60% of freckle patients report a family history; freckles are common across Asian, European, and mixed-heritage backgrounds.
UV radiation — UVA / UVB (primary aggravating factor)
UV UV radiation directly stimulates genetically susceptible melanocytes to overproduce melanin. This is why freckles darken in summer and fade in winter. Even on cloudy days, through windows, or indoors, UVA can still trigger this response.
High UV exposure during childhood (sensitive developmental period)
5–10 During childhood, melanocytes are highly active, and insufficient sun protection early in life can significantly increase freckle count and density. This pattern is particularly common among clients who grew up in Toronto or had extensive summer sun exposure before immigrating.
Fitzpatrick I–III Higher risk in lighter skin tones
Freckles are most common in clients with lighter skin tones (Fitzpatrick I–III), and are actually less prevalent in deeper skin tones (Fitzpatrick V–VI). Most Asian women fall within Fitzpatrick III–IV, placing them at higher incidence than those of deeper Asian or South Asian heritage.
Seasonal variation (fades in winter, darkens in summer)
Seasonal variation is a key diagnostic marker for freckles. If your spots darken noticeably in summer (Toronto, May–September) and fade significantly in winter (November–March), this is a classic feature of freckles — distinguishing them from melasma (present year-round) and sunspots (which do not fluctuate with the seasons).
Freckles vs. Melasma / Sunspots / Moles / Post-Acne Marks
Many clients arriving for a consultation at ID Cosmetic Clinic (Toronto / Markham) refer to all facial spots simply as “spots” — yet several clinically distinct pigmentation conditions can look nearly identical,Treatment protocols are entirely different. Dr. Xu uses a three-dimensional diagnostic approach — Wood’s lamp, dermascopy, and Fitzpatrick skin typing — for precise differentiation.

What makes freckle treatment different for Asian skin?
Although freckles are most common in clients with lighter skin tones (Fitzpatrick I–III),Freckle treatment in Asian skin (Fitzpatrick III–V) requires its own unique considerations.. This is a core strength of ID Cosmetic Clinic — over 90% of our 5,000 clients · 80,000 treatment areas involve Asian clients.
1. Freckles in Asian skin frequently co-exist with other pigmentation conditions
Among Asian female clients in Toronto / Markham,Freckles frequently co-exist with melasma, sun spots, and post-inflammatory hyperpigmentation (PIH). Clinically, more than 30% of clients presenting with freckles are found during consultation to have undetected early melasma or post-acne pigmentation as well. This is why precise subtyping with a Wood’s lamp before treatment is essential — different lesionsseparate protocols.
2. Freckles in deeper skin tones (Fitzpatrick IV–V) require a cautious starting approach
Although the risk of post-inflammatory hyperpigmentation (PIH) from freckle treatment itselfsignificantly lower than with melasma, That said, clients with deeper Asian skin tones (Fitzpatrick IV–V) may still experience temporary pigmentation changes with high-energy laser treatments. Dr. Xu’s protocol:
- Fitzpatrick IV–V clients undergo a test spot first
- Starting at low-to-moderate energy settings, then safely and gradually increasing
- Mandatory SPF 50+ sun protection for 4 weeks post-treatment
- Avoid peak summer UV exposure (Toronto: June–August)
Under Dr. Xu’s protocol, the post-inflammatory hyperpigmentation (PIH) rate for freckle treatments in Fitzpatrick IV–V clients is <1%.
3. 「”one precise treatment” principle — freckles can be cleared in a single session
This is the key distinction between freckles and melasma — Epidermal freckles have a straightforward structure, PicoSure Pro 755nm Targeted spot treatment can achieve significant clearance in just 1–2 sessions. Dr. Xu does not recommend spreading treatment across 4–5 “conservative” sessions — doing so only makes long-term sun protection maintenance more challenging.
Fitzpatrick Grading × Freckles Treatment Strategy
| Fitzpatrick | Skin tone | PIH risks | Dr. Xu’s Protocol |
|---|---|---|---|
| I–II | fair / fair-skinned | extremely low | Standard-energy PSP × 1 session + SPF |
| III Light Asian | Beige | Low | Medium-to-standard energy × 1–2 sessions + SPF |
| IV Typical Asian | golden-yellow | Low–Moderate | Moderate energy × 1–2 sessions + test spot + SPF |
| V Deeper Asian / South Asian | Light brown | Medium | Low-energy start × 2 sessions + test spot mandatory |
Freckles are among the laser aesthetic treatments whereBest return on investment— one treatment, visible results, virtually minimal downtime, maintained with sun protection. The key prerequisite: the physician must correctly distinguish true freckles from other pigmentation and accurately calibrate the starting energy parameters for your Fitzpatrick skin type. —— Dr. Dan Xu, M.D., CCFP
Freckles Treatment Protocol (Devices + Sun Protection)
The primary treatment for freckles isTargeted laser spot treatment + Long-term maintenance with SPF 50+ sun protection. Unlike melasma, frecklesNo topical medication required alongside treatment. Dr. Xu designs your treatment protocol based on your Fitzpatrick skin type, the density and distribution of your freckles, and your prior treatment history, selecting from the modalities below.
PicoSure Pro® 755nm Picosecond laser first-line choice for freckles
Freckles’Gold standarddevice. The 755nm wavelength delivers the strongest epidermal pigment absorption, and the picosecond pulse width (550ps) works byphotoacoustic effectInstantly shatters melanin particles with minimal thermal damage. For freckles on Asian skin, 1–2 sessions can achieve significant clearance, FOCUS The FOCUS lens array can simultaneously improve pore appearance and overall skin tone. For more information, see PicoSure Pro treatment page. For lighter brown freckles, Dr. Xu may use the 532 nm wavelength of the PicoWay laser instead.
- 1–2 Results visible after 1 session (most clients achieve 80%+ clearance in a single session)
- Recovery period 24–72 hours (crusts fall off naturally)
- Fitzpatrick I–V Safe
- Health Canada dual certification
PicoSure Pro FOCUS FOCUS lens array (full-face brightening adjunct)
Clients seeking freckle treatment often wish to address overall skin tone and pore appearance at the same time. The FOCUS lens array can be applied after targeted spot treatment toFull-face uniform treatment, Stimulates collagen regeneration and promotes even pigmentation. Minimal downtime — performed in the same session as targeted spot treatment.
- Same-session add-on, 15 additional minutes
- Overall skin brightening + pore refinement
- Fitzpatrick III–V Safe
Ai BBL® HEROic™ IPL (Diffuse freckles (adjunct treatment)
Sciton Smart IPL, suitable forDiffuse freckles (Fitzpatrick I–III) in fair-skinned patientsHighly effective for treating scattered small freckles across the full face in a single pass.Fitzpatrick IV High-energy IPL is not recommended for darker Fitzpatrick skin types, Dr. Xu will recommend PicoSure Pro as an alternative.
HydraFacial / Medical-grade skincare (recovery support)
Can be performed 2–3 weeks post-treatment HydraFacial Helps accelerate crust shedding and replenishes moisture to support barrier repair. Not a primary freckle treatment — used solely as a post-procedure soothing adjunct.
SPF 50+ Sun protection + antioxidant skincare cannot be skipped
No freckle treatment can deliver lasting results without adequate sun protection. Dr. Xu’s protocol always includes:
- Daily SPF 50+ mineral or chemical sunscreen (reapply every 2 hours / wear a hat outdoors)
- Antioxidant serum (Vitamin C / E / resveratrol)
- Oral sun protection supplement (such as Heliocare) — intensified during summer months
- Avoid deliberate sun exposure (Minimize outdoor exposure when Toronto’s summer UV index reaches 8+
Protocol Recommendations by Freckle Type
| Client Type | Recommended Protocol |
|---|---|
| Classic nose-and-cheek distribution · Fitzpatrick III | PicoSure Pro Targeted spot treatment × 1 session + long-term SPF 50+ |
| Dense distribution · Scattered across the entire face | PicoSure Pro Targeted spot treatment + FOCUS lens array × 1 session (same visit) |
| Fitzpatrick IV–V Deeper skin tone | Test spot → Medium-to-low energy PSP × 2 sessions + SPF |
| Freckles + melasma overlap | Freckle targeted PSP treatment × 1 session → separate melasma protocol |
| Sun-exposed areas: neck, chest, and backs of hands | Targeted spot treatment × 1–2 sessions + lifelong sun protection |
How does Dr. Xu develop a freckle treatment plan? (6-Step Process)
This is the fundamental difference between ID Cosmetic Clinic and a typical medspa in Toronto / Markham — the same freckles, treated with the right protocol, can clear in a single session. The wrong device choice risks misidentifying melasma as freckles and making the condition significantly worse.
Accurate diagnosis (first confirming whether the spots are truly freckles)
- Wood’s lamp Examination —— Epidermal freckles fluoresce brightly; mixed-type melasma shows uneven enhancement
- Dermoscopy —— Evaluating pigment structure and border characteristics
- Fitzpatrick Classification / Subtyping —— Determining skin tone risk level
- Medical History Assessment —— Childhood onset, family history, worsening in summer
- Photo documentation on file —— Baseline photos taken under standardized lighting for future comparison
Distinguishing pure freckles from mixed pigmentation conditions
30%+ The client believed she only had freckles when she arrived at ID Cosmetic. In reality:
- Freckles only → PicoSure Pro targeted spot treatment, 1–2 sessions
- Freckles + early melasma → separate protocols (freckles addressed in 1 session; melasma managed gradually)
- Freckles + sun spots → targeted PSP treatment in a single session
- Moles misidentified as freckles → referred for surgical evaluation; laser treatment is not appropriate
Assess PIH risk level
PIH risk associated with freckle treatmentOverall very low, However, clients with Fitzpatrick IV–V skin, recent sun exposure, or a history of post-inflammatory hyperpigmentation (PIH) still require a test spot evaluation. In Dr. Xu’s protocol, the presence of any single high-risk factor warrantsTest spot mandatory (Small test spot behind the ear, observed over 4 weeks).
Designing a treatment timeline (freckle-specific)
The freckle treatment timeline is far simpler than that of melasma, howeverTiming and season selection are key:
[Weeks 0–4]SPF 50+ Strict sun protection leading up to treatment (avoid pre-treatment tanning)
[Week 4]Test spot (high risk) or proceed directly to treatment
[Weeks 4–5]PicoSure Pro 755nm Targeted freckle treatment (45–60 minutes)
[Week 5–6]Scabbing phase (naturally sheds within 5–10 days)
[Week 8]4 Weekly follow-up + photographic comparison
[Week 10–12]2nd session (if residual pigmentation >20%)
[long-term]Annual late-summer review (October) + lifelong SPF 50+
Optimal treatment window:Late autumn to early spring (October–March), Toronto’s UV index is lower during this period, making it the optimal time for post-treatment maintenance.
Post-treatment follow-up and refinement
- 4 Week follow-up (Photographic comparison and clearance rate assessment)
- Scab management (do not pick or scratch; gentle cleansing; use of medical-grade dressings)
- Sun protection check (inquire about product options and reapplication frequency)
- Touch-up session as needed (For patients with >20% residual pigmentation, a second session is scheduled at 6 weeks.)
long-term maintenance (freckles are a low-maintenance condition)
Maintaining freckle results is far simpler than managing melasma.. Full protocol:
- SPF 50+ Year-round sun protection (daily + outdoors + cloudy days + through windows)
- Annual follow-up once per year in late summer (October) (to check for any newly developed freckles)
- Low-energy maintenance session once every 1–2 years as needed (individuals with high UV exposure)
- Antioxidant skincare (Long-term use of vitamin C serum)
Freckle treatment is one of the areas most prone to over-promising — “permanent elimination” simply does not exist, because the genetic predisposition remains. However,「”Clear now, maintain with lifelong sun protection”is a very achievable goal. —— Dr. Dan Xu, M.D.
How soon will I see results from freckle treatment? How many sessions are needed? Can freckles come back?
The greatest advantage of freckle treatment isFast results + low recurrence rate. The three tables below answer the three most frequently asked questions.
How soon will I see results?
| stage | Duration | Visible improvement |
|---|---|---|
| Immediately post-treatment | 0–24 hours | mild redness and frosting reaction at treated spots |
| Scabbing phase | 3–10 days | Treated spots darken, form natural crusts, and may show slight flaking |
| After scabs have shed | 2–4 weeks | Significant clearance: 80%+ of freckles visibly eliminated or substantially faded |
| 1 Assessment at 6 weeks after each session | 6–8 weeks | Long-term results are stable: single-session clearance rate of 80–95% |
How many sessions are needed?
| Freckle types | Recommended Sessions | Interval |
|---|---|---|
| Classic nasal-cheek distribution · fair | 1 sessions | — |
| Dense distribution · Moderate | 1–2 sessions | 6 weeks |
| Deeper skin tone · Fitzpatrick IV–V | 2 sessions | 6–8 weeks |
| Sun-exposed areas: neck, chest, and backs of hands | 1–2 sessions (by zone) | 4–6 weeks |
Can freckles come back? Recurrence rates (reference statistics)
| Maintenance | 12 recurrence rate within months |
|---|---|
| Strict SPF 50+ application + antioxidant skincare | 5–15% (mostly new freckles forming elsewhere; original treated spots do not return) |
| Basic sun protection (SPF 30, occasional reapplication) | 20–35% |
| No sun protection at all, with frequent outdoor activity | 50%+ (new freckle formation becomes apparent) |

Why Do MedSpas Get Freckle Treatment Wrong? Real Cases
Client B — Asian female, age 30 · Fitzpatrick IV · Scattered freckles across the nose and cheeks + early-stage melasma over the zygomatic area (undetected by the client prior to her consultation at ID Cosmetic)
going to a MedSpa Treat on sight · No diagnostic evaluation

- No test spot performed, no Fitzpatrick assessment conducted
- no post-treatment follow-up, no sun protection guidance provided
- Outcome: a patient who came in for freckles left with a new, significant melasma flare-up
- Regret → returning to ID Cosmetic for melasma revision treatment (at additional time and cost)
at ID Cosmetic Clinic Precise diagnosis · Separate protocols

Freckles Treatment: Common Risks, Contraindications & Safety Protocol
The overall risk profile for freckle treatment is very low — but transparency is a mark of clinical integrity. We openly share all potential reactions, serious risks, contraindications, and absolute exclusions so you have complete information before your consultation.
Common reactions (in the short term after treatment)
- Post-laserRedness and warmth (1–4 resolves within hours; cold compress may be applied)
- Frosting response at treated spots (frosting) —— Immediate whitening at treated spots, darkening within a few hours
- Scabbing phase —— Treated spots turn into small brown-black dots,5–10 naturally sheds within days (do not pick or scratch)
- Mild flaking (as scabs naturally shed)
- Skin sensitivity and dryness (1–2 weeks; mandatory SPF 50+ and gentle moisturization)
Serious risks (rare, but require informed disclosure)
- Post-inflammatory hyperpigmentation (PIH) —— Extremely rare (< 1%), most often seen in Fitzpatrick IV–V skin types with inadequate sun protection
- Hypopigmentation (Hypopigmentation, rare with picosecond technology)
- Misidentifying moles as freckles and treating them as such (when no pre-procedure dermoscopy assessment was performed)
- Unidentified co-existing melasma → worsened by high-energy treatment
- Scarring (extremely rare with PicoSure Pro; more commonly associated with improper aftercare or picking at scabs)
Relative contraindications (require careful assessment)
- Active tanning or unresolved sun exposure within the past 2 weeks
- Currently using photosensitizing medications (e.g., tetracycline, sulfonamides, St. John’s Wort)
- Active herpes simplex (cold sores)
- Unclassified pigmentation (dermascopy required before treatment)
- co-existing uncontrolled melasma

Toronto Freckles Treatment FAQ
Answered personally by Dr. Dan Xu — based on the most frequently asked freckle questions from patients in Toronto and Markham. What Is the Most Effective Treatment for Freckles?
Can freckles be permanently eliminated? Will they come back?
What is the difference between freckles and melasma?
How many days of downtime should I expect after freckle treatment?
Is freckle treatment safe for Asian skin? Can it cause post-inflammatory darkening?
What is the best season for freckle treatment?
Is freckle treatment painful? Is anesthesia needed?
Why did my freckles get worse instead of clearing after IPL treatment?
How is freckle treatment priced? Why do costs vary so widely between clinics?
Price differences between clinics are not about the device itself, but aboutAssessment depth。Freckles may appear straightforward, but accurate diagnosis first requires Wood's lamp and VISIA imaging to differentiate pure freckles, freckles overlapping melasma, Hori's nevus, and other pigmentation types. This step demands a physician consultation — it cannot be delegated to a technician — and it is the fundamental difference between a physician-led clinic and a standard medspa.
Straightforward, isolated, and stable freckle cases may be performed by our clinical team following Dr. Xu's PicoSure Pro Zoom Lens standard protocol, offering greater accessibility. Cases involving co-existing melasma, recurrent post-inflammatory hyperpigmentation (PIH), prior treatment failure, or atypical distribution are managed directly by Dr. Xu, including pre-treatment conditioning and real-time parameter adjustments.Treatment plans and pricing are determined by your individual condition — not by a standard price list.。New clients receive a complimentary 30-minuteFreeDuring your consultation, Dr. Xu will perform a Wood's lamp assessment and walk you through a personalized discussion based on your pigmentation type.
