Quick Answer

Turkey neck, or neck laxity, refers to loose, sagging skin and weakened platysmal bands along the neck that create a drooping appearance often associated with aging or significant weight loss. Non-surgical treatments — including radiofrequency tightening, ultrasound-based lifting (Ultherapy), and injectable neuromodulators — can visibly improve skin firmness and neck contour without surgery. At ID Cosmetic Clinic in Markham and Toronto, Dr. Dan Xu, M.D., personally assesses and performs each treatment. A series of one to three sessions is typically recommended, with gradual results developing over two to six months.

WHAT IS TURKEY NECK

What Is Turkey Neck?

Turkey Neck(turkey neck)is a colloquial term; medically referred to asCervical Aging Syndrome, refers to neck changes caused bydermal collagen loss, platysma laxity, subcutaneous fat redistribution, and excess skinacting together to produce a combined appearance of skin sagging, vertical bands, and horizontal wrinkles. The term comes from its resemblance to the loose, hanging skin of a turkey’s neck.

The 5 Anatomical Layers of Neck Aging

1

Epidermis — dryness and pigmentation

Chronic UV exposure combined with neglected sun protection leads to epidermal thinning, pigmentation (particularly on the sides of the neck), and rougher skin texture.Neck SPF is overlooked by almost all patients.

2

Dermis — loss of collagen and elastin

40 After this age, dermal collagen decreases by approximately 1% per year, while elastin begins to degrade even earlier. Because the neck’s dermis is naturally thinner than that of the face, collagen loss has a more visible impact on appearancemore pronounced than on the face.

3

Subcutaneous fat — loss and redistribution

Loss of superficial neck fat causes the skin to”Volume loss”;Deep fat in the submental and jawline region may accumulate simultaneously, contributing to the appearance of a double chin.the contrast between upper and lower — face and neckworsening the appearance of aging.

4

Platysma — Muscle Tension Imbalance

The platysma is a broad, thin muscle running across the front of the neck. With age, uneven tension develops, resulting inVertical bands (“Turkey Neck”). Most noticeable when speaking or tensing the neck.

5

SMAS / Deep structural support

The SMAS fascia along the jawline is continuous with the platysma; deep-layer aging causes the jawline definition to become”downward pull”,creating a cascade effect of jowls + neck laxity.Neck rejuvenation must address the jawline together.

4 Key Signs of Neck Aging

  • neck lines(Tech Neck / Necklace Lines) — 1–4 horizontal creases across the front of the neck
  • Platysmal bands(Platysmal Bands) — 1–3 vertical muscle bands across the anterior neck, becoming more prominent when speaking or looking down
  • Skin laxity and sagging(Skin Laxity) — The skin along the front of the neck loses tension; in advanced cases it sags like”turkey neck”
  • Loss of jawline definition(Jawline Blurring) — Loss of definition between the jawline and neck, resulting in an overall puffy appearance

3 common neck aging patterns

Early-Stage (Tech Neck)

Early Stage · Tech Neck Type

30–40 years old, with neck lines as the primary concern and reasonably good skin laxity. Commonly seen in individuals who spend extended hours looking down at phones or computers. Skinvive + micro-filler can effectively improve this presentation.

Moderate (platysmal band type)

Moderate · Platysmal band type

40–55 Prominent platysmal bands with early sagging. The Nefertiti Lift (neuromodulator) combined with Sofwave™ energy-based treatment is the preferred combination approach.

Severe (sagging type)

Severe · Generalized Laxity Type

55+ years of age, with excess skin laxity, a double chin, and a lost jawline definition. A multi-modality combination approach with a long-term, staged treatment plan is required. In the most severe cases, surgical neck lift should be considered.

5 steps for a preliminary self-assessment

  • Stand tall and look straight into a mirror — do you notice anyvisible horizontal creases?
  • When speaking or tensing the neck, do you noticeVertical bands?
  • When you look down, does the neck skinVisible product buildup?
  • The relationship between the jawline and the neckBoundaryIs it becoming less defined?
  • Side-profile view of the neckAngleIs it < 105° (ideal range: 105–120°)?

3 or more indications”yes”,Visible neck aging is present. Accurate assessment requires palpation of skin tension, evaluation of the platysma, and multi-angle photography.

CAUSES & TRIGGERS

Common Causes of Turkey Neck (6 Key Contributing Factors)

Neck aging is the result of multiple overlapping factors — natural aging is only one piece of the picture, and the impact of lifestyle and environmental influences is often underestimated.

01

Age-related natural collagen loss

Dermal collagen decreases by approximately 1% per year after age 25, accelerating after menopause. The neck’s baseline dermisNaturally 30% thinner than facial skin, loss has a more pronounced impact on appearance.

Clinical significance:40 Starting preventive energy-based treatment with Sofwave™ every 18–24 months can delay visible aging of the neck by 5–10 years.
02

Chronic forward head posture (Tech Neck)

Looking down at phones and tablets repeatedly folds the skin along the front of the neck, accelerating the formation of horizontal lines. People who spend 3–4 cumulative hours per day in this position may develop noticeable neck lines as early as age 30.

Clinical significance:Treatment of horizontal neck lines requiresalongside changing forward-head habits(elevating your screen, scheduling regular neck extension stretches) — without these habits, recurrence is inevitable.
03

Photoaging (UV radiation)

The neck — particularly the sides and back — is one of the most severely sun-damaged areas of the body, largely because SPF protection is chronically neglected there. UV exposure directly degrades collagen, leading to pigmentation and rough skin texture.

Clinical significance:Apply SPF to your neck every day and reapply outdoors — this is the most affordable and effective neck anti-aging step you can take.
04

weight fluctuations / rapid weight loss

Significant weight fluctuations cause the neck skin to repeatedly stretch and retract, leading to cumulative damage to elastic fibers.40+ Rapid weight lossneck laxity often appears earlier than expected.

Clinical significance:Incorporating Sofwave™ energy treatment alongside a weight-loss journey can significantly reduce the risk of skin laxity.
05

Platysma hyperactivity

Some individuals are born with naturally higher platysmal tone, causing visible anterior neck bands when speaking or under stress. Genetics, chronic tension, and poor head-and-neck posture all contribute.

Clinical significance:Nefertiti Lift(neuromodulator) is the most direct and effective approach.
06

hereditary skin laxity

A family history of premature aging or skin laxity often means neck aging appears earlier and more pronounced. Genetics govern collagen quality and metabolic rate.

Clinical significance:Clients with a positive family history of premature neck aging are advised to begin preventive neck care from age 35.
DIFFERENTIAL DIAGNOSIS

Turkey Neck vs. Neck Lines / Double Chin / Jawline Laxity

Many clients present with”neck aging”,but the specific concerns differ, and so do the treatment approaches. Misdiagnosis leads to spending money without addressing the root issue.

vs Isolated horizontal neck lines

NECKLACE LINES
fundamentallyDermal creasing + partial muscle contraction
Appearance1–4 horizontal neck creases
Palpationnormal skin laxity
Treatment planSkinvive + Micro-filler + Sofwave™
Prognosis3 noticeable improvement by month

vs double chin

SUBMENTAL FAT
fundamentallySubmental fat accumulation
AppearanceBelow the jawline”Second layer”
Palpationsoft and pinchable
Treatment planBelkyra Fat dissolving + energy-based radiofrequency
Prognosis2–4 sessions, optimal results at 9 months

vs Jowling (sagging along the lower face)

JOWLS
fundamentallySMAS ptosis + retaining ligament laxity
AppearanceBoth sides of the jawline”pouch”
Palpationcontaining soft tissue + fat
Treatment planSofwave + Mid-face filler support
PrognosisA comprehensive facial treatment plan is needed

vs Platysmal bands

PLATYSMAL BANDS
fundamentallyExcessive longitudinal tension in the platysma
Appearance1–3 vertical bands visible when speaking
PalpationMuscular in origin, becoming more prominent under tension
Treatment planNefertiti Lift Neurotoxin
Prognosis2 weeks to take effect, results maintained for 4 months
Most clients 40+ present with a mixed type — Neck lines, platysmal bands, and early laxity are present simultaneously. During your consultation, Dr. Xu will provide a clear, layered diagnosis and prioritize your treatment sequence accordingly.
ASIAN NECK AGING PATTERN

Why Does Neck Aging Present Differently in Asian Patients?

The pattern of neck aging in Asian faces differs significantly from that seen in Western populations. Applying Western treatment protocols directly often yields suboptimal results — or can even be counterproductive.

1. Neck skin tends to be fairer but thinner

The neck skin of Asian women tends to befair-toned but with a thinner dermis,Collagen density is lower than in Caucasian populations. This means:

  • Horizontal neck lines appear earlier (visible as early as age 30)
  • Energy-based devices require the use ofMore precise treatment parameters,Avoid burns
  • Filler dosage should be more conservative

2. Neck aging ≠ facial aging timeline

Many Asian women in their 40s still have a youthful face — thanks to genetics and diligent sun protection — yet the neck shows significant signs of aging.“face at 35, neck at 50”This is one of the most common presentations we see at ID Cosmetic Clinic — and in these cases, neck rejuvenationshould be prioritized over continuing facial treatments.

3. “Tech Neck” more pronounced in Asian patients

The combination of prolonged desk work and heavy smartphone use in urban Asian lifestyles means Tech Neck is significantly more prevalent here than in Western populations.30+ Horizontal neck lines appearingis the norm among urban Asian women.

Many Asian women ask me:”Doctor, my face looks fine — it’s just my neck that’s aging.” My answer is:The neck is your”second face”. Among my 8,000+ complex revision cases, concern about neck rejuvenation has risen significantly in recent years — because clients are finally recognizing that even a perfectly treated face can appear 5–10 years older when the neck is left lax and untreated. — Dr. Dan Xu, M.D., CCFP
TREATMENT PROTOCOL

Turkey Neck Treatment Options (Energy-Based, Injectable & Combination)

There is no”a single optimal solution” — A multi-modal protocol must be tailored based on the degree of aging, skin laxity, muscle tone, and fat distribution. Energy-based options fall into two families that are not interchangeable: radiofrequency skin tightening and ultrasound lifting. Which family fits depends on the tissue layer involved, not on which device is best known.

01

Sofwave® Neck tightening Preferred energy-based treatment

FDA authorizedSynchronized Ultrasound Parallel Beam (SUPERB) technologyA non-invasive technology that stimulates deep dermal collagen regeneration. Dr. Xu is a Sofwave Medical Canada KOL with extensive experience applying Sofwave™ to neck rejuvenation.

  • Single treatment session, minimal downtime
  • 3 months to initial results · optimal at 6 months
  • Maintenance: 18–24 months
  • Neck + jawline combination yields optimal results
02

Nefertiti Lift(platysma neuromodulator) first-line choice for platysmal bands

Botox Injected into the platysmal bands and along the inferior jawline,Relaxing the downward pull of the neck muscles, allowing the jawline to”Released / restored”. A typical dose of 20–40 units provides results lasting 3–4 months. This injection requires advanced technique and a thorough understanding of platysma anatomy.

03

Skinvive Neck micro-droplet injection Top choice for hydration

Allergan Skinvive series forMicro-droplet injection techniqueImproves neck dryness, fine lines, and overall hydration. A course of 3 sessions, spaced 4 weeks apart, with results maintained for up to 6 months. An ideal first-choice preventive treatment for younger patients.

04

Neck line micro-filler (Volbella) Preferred for deeper lines

Volbella XC Ultra-precise micro-injections administered into the dermal layer of neck lines,Direct volumizationDeep, stubborn lines. Important: dosage must remain conservative (0.05–0.1 ml per line) to avoid nodule formation.

05

Belkyra Submental fat dissolving double chin

FDA Health Canada-authorized deoxycholic acid injection,Permanent dissolutionsubmental fat. 2–4 treatment sessions, spaced 4–6 weeks apart. Swelling under the chin may persist for 1–2 weeks post-injection. An essential option for neck aging accompanied by a double chin.

06

HydraFacial Neck + chemical peel Surface-level treatment

For clients with neck pigmentation, rough texture, or visible pores, a combination of a HydraFacial® neck-specific protocol and a gentle mandelic acid peel can be added to the treatment plan.High-energy IPL is not used on the neck(higher risk of pigmentation).

07

At-home skincare + SPF cannot be omitted

In-clinic treatment accounts for only 30% of long-term neck rejuvenation — the remaining 70% depends on your at-home routine:

  • Daily SPF 50+(wherever you apply skincare on your face, apply it to your neck too)
  • Dedicated neck cream(containing retinol / peptides / ceramides)
  • Avoid prolonged head-down posture(raise your screen to eye level + perform regular neck extension stretches)
  • Proper sleep position(avoid high pillows)

DR. XU’S 6-STEP PROTOCOL

How Does Dr. Xu Build a Neck Treatment Plan? (6-Step Process)

Treating neck aging requires”Layered diagnosis + multi-modal combination + long-term maintenance”.

1

Multi-angle photography + dynamic assessment

  • Front-facing, 45°, and 90° views Three-angle photography
  • Tilting the head back / looking down / tensing the neck Dynamic video
  • measurement data:Neck angle, number of horizontal lines, and number of platysmal bands
  • Palpation:skin tension, fat distribution, and muscle tone
2

layered diagnosis

Neck aging must be clearly identified”which layer — or which layers”is the primary cause:

  • Epidermis (pigmentation / texture) → resurfacing + HydraFacial®
  • Dermis (collagen loss) → Sofwave™ + Skinvive
  • Platysma (muscle bands) → Nefertiti Lift
  • Fat (double chin / subcutaneous volume loss) → Belkyra® / filler
  • SMAS(Deep laxity) → multi-modal treatment / surgical referral
3

assessing facial continuity

The neck cannot be assessed in isolation — it must be considered alongsideJawline + mid-faceAssessed together. In over 70% of neck aging cases, the jawline also requires concurrent treatment.

4

designing a phased treatment timeline

Neck rejuvenation ismulti-phase treatment plan,not a one-session fix:

[Week 0]Consultation + layered diagnosis + treatment plan finalization
[Phase 1]Sofwave + Nefertiti Lift(can be done same day)
[Weeks 4–8]Skinvive × 2–3 sessions
[Week 12]3 Monthly assessment · micro-filler fine-tuning
[Week 16]Nefertiti maintenance
[Week 24]6 month optimal outcome assessment
[Annual maintenance]Sofwave Every 18 months · Nefertiti Lift every 4 months · Skinvive every 6 months
5

at-home regimen carried out concurrently

  • SPF 50+ Daily neck
  • Retinol neck cream (introduce gradually to build tolerance)
  • Raise your screen to eye level + perform regular neck extension stretches
  • Sleeping with a low pillow
  • Avoid significant weight fluctuations
6

Long-term maintenance (essential!)

neck rejuvenationWithout”a permanent, one-and-done solution”:

  • Sofwave Once every 18–24 months
  • Nefertiti Every 3–4 months
  • Skinvive Every 6 months
  • Annual reassessment of overall face-to-neck harmony
  • Dynamic plan adjustments (based on aging progression)
neck rejuvenation”Pitfall”is — Immediate results are not visible after a single treatment, clients can easily feel disappointed. However, cumulative results at 6 months and 1 year are highly significant. Both the physician and the client need to approach this with patience. — Dr. Dan Xu, M.D.

WHY THIS MATTERS

Neck rejuvenation is not”running a device over the neck”, but rather “Layered diagnosis + multi-modal protocol”

The same Sofwave device can yield vastly different results depending on the treatment protocol, depth settings, and energy parameters chosen by the physician. As a Sofwave Medical Canada KOL, Dr. Xu’s neck rejuvenation methodology serves as a reference standard for peer training.

PRICING PHILOSOPHY

About the Cost of Neck Rejuvenation

Neck rejuvenation is not like injectable treatments —”How much is one vial/unit?”That straightforward — because itis fundamentally a 6–12 month program involving a combination of multiple devices and injectables. Even when two clients present with the same chief concern,”Neck laxity”,One client may need only Skinvive plus micro-filler, while another may require a triple-modality combination of Sofwave™, Nefertiti Lift, and Belkyra® — each plan differs substantially in clinic time, depth of physician assessment, device usage, and number of follow-up visits.

At ID Cosmetic Clinic,Multi-modality neck rejuvenation protocols are designed and led by the physician. Injectable treatments — including Nefertiti Lift neurotoxin, neck line micro-filler, Skinvive, and Belkyra — are typically performed directly by the physician. The neck’s complex vascular anatomy means that platysma injections demand precise dosing and depth control. For energy-based treatments such as Sofwave™, the physician designs all treatment parameters, while the trained medical team carries out the procedure under physician supervision — a distinction that affects the overall cost structure. Routine annual maintenance sessions can be scheduled more efficiently, whereas complex revision cases, significant laxity, or clients with special medical considerations are best managed directly by the physician.deeply involved throughout the entire process.

We do not publish a standardized price list online, because neck rejuvenation”Pricing”Discussing pricing without a treatment plan is inherently misleading. Following a consultation assessment, Dr. Xu will provide a clear, personalized multi-phase plan with a corresponding cost structure based on your layered neck diagnosis. We recommend approaching neck rejuvenation as“Annual wellness investment”Rather than”Per-session cost”approach — this leads to more rational decision-making and greater long-term value.

Can vertical neck bands be fixed with a single injection?

Dynamic-type cases can show significant improvement with a single treatment session, but laxity-predominant cases cannot.Clients with purely dynamic, hypertonic platysmal bands will see a noticeable reduction in band prominence within 7–14 days of Botox® / Dysport® micro-droplet injection. However, clients over 45 who also present with skin laxity and dermal thinning must first undergo energy-based lifting with Ultherapy® or Thermage® FLX before considering muscle relaxation — otherwise, injection alone will deliver disappointing results.Your classification determines your treatment plan — it's not simply "inject vs. don't inject"。

Is platysma injection safe? Could it cause difficulty swallowing?

in appropriate dosage + precise injection depth + extensive physician experience— platysma injection is a very safe aesthetic procedure. Swallowing discomfort arises primarily from three causes: (1) excessive single-session dosage; (2) injection depth reaching the deep surface of the sternocleidomastoid or the infrahyoid muscle group; (3) insufficient operator experience. Dr. Xu follows a Korean micro-droplet multi-point protocol (8–12 injection points per band / 1–2 U per point / ultra-superficial depth), and swallowing discomfort events are extremely rare in clinical practice.

Botox Botox® vs. Dysport® for the neck — which is better?

Both are Health Canada–licensed type-A botulinum toxins with the same mechanism of action. The differences lie in:Dysport slightly higher diffusion, faster onset (3–5 days vs. 5–7 days for Botox®),Dysport® is better suited to clients with broader platysma coverage and multiple bands. Botox® has a smaller diffusion radius and greater precision, making it preferable for clients with compact anatomy who require fine-grained control. Dr. Xu selects between them based on your individual platysma distribution — there is no universally superior option.

Do I still need Botox® after Ultherapy®?

It depends on your classification.pure laxity typeFor patients who have completed Ultherapy® and find that vertical bands are no longer visible at rest or with movement, neuromodulator injection may not be necessary. However, combination typeFor patients with combined laxity and muscle hyperactivity, vertical neck bands may still become prominent with movement even after Ultherapy® — a small amount of Botox® is then needed to fully address the dynamic banding. Our approach is to complete the energy treatment first, reassess at 4 weeks, and then decide whether injection is warranted. We do not stack everything in a single session.

How many sessions does turkey neck treatment require? How often is maintenance needed?

Depends on subtype:dynamic typeA single session per treatment cycle, maintained every 4–5 months;Laxity-dominant combined typeThe primary treatment phase consists of 1 session of Ultherapy® + 1 session of Botox®; maintenance follows with Ultherapy® every 12–18 months and Botox® every 4–5 months.combination typerequires a combination of Botox® + Microbotox + Profhilo, maintained every 4–5 months. All treatment plans require2 One follow-up visit per week for minor adjustments。

Can vertical neck band treatment be performed during pregnancy or breastfeeding?

No.Safety data for botulinum toxin during pregnancy and breastfeeding is insufficient — all neuromodulators are explicitly contraindicated in these periods. While there is no direct evidence of harm from energy-based devices such as Ultherapy® and Thermage® FLX, we recommend postponing treatment until at least 3 months after complete weaning as a precautionary measure. In the meantime, you can focus on daily physical sun protection and a targeted neck skincare routine.

Will my speech or swallowing be affected after treatment?

Under Dr. Xu's micro-droplet multi-point protocol,The vast majority of patients experience no functional impact。A small number of clients may notice a mild sense of tightness in the neck or slight looseness when tilting the head back during the first 1–2 weeks after treatment. This is a normal response to reduced platysma tone and typically resolves with adaptation. If you experience noticeable difficulty swallowing or weakness when lifting your head, please return for evaluation immediately — we have a complete protocol in place for this situation.

Does treating vertical neck bands require surgery for lasting results?

Not necessarily. Most patients should begin with a non-invasive combination approach(muscle relaxation + energy-based tightening + dermal quality improvement), with consistent maintenance over 1–2 years, the neck can reach and sustain a noticeably improved condition. Only a very small number ofSevere skin laxity + significant platysma ptosis + submental fat accumulationfor whom non-invasive options can no longer meet aesthetic goals — surgical neck lift assessment becomes appropriate. Dr. Xu will help you determine which category you fall into during your consultation.

Can vertical neck bands and horizontal neck lines be treated at the same time?

Yes, but it is essential to understand that the two conditions are fundamentally different.vertical bands are a muscle issue, with platysma micro-droplet multi-point injection;Horizontal lines are a dermal crease issue, using Microbotox (micro-droplet neuromodulator) + Profhilo / skin booster + a small amount of filler if needed. For combination-type patients, we design a comprehensive plan in a single consultation and execute it in stages or in the same session, depending on dosage and recovery considerations.
