Quick Answer

Tear troughs are the sunken grooves that form beneath the eyes where the lower eyelid meets the cheek, often creating a tired or aged appearance. Hyaluronic acid filler is a common non-surgical option suited for adults bothered by under-eye hollowness who have realistic expectations. Treatment works by gently adding volume to the tear trough area to soften the shadow. At ID Cosmetic Clinic in Markham and Toronto, tear trough filler is personally performed by Dr. Dan Xu, M.D., CCFP. Most patients see results in one session, with effects typically lasting nine to twelve months.

WHAT IS TEAR TROUGH

What are tear troughs? Why are they so often confused with dark circles and under-eye bags?

The tear trough (medically known as the nasojugal groove) is a concave groove located along the inner lower eyelid that extends along the inferior border of the orbicularis oculi muscle toward the mid-cheek.depressed groove。Its nature is not truly “dark” — rather it isanatomical hollow —— The retaining ligaments pull the skin toward the periosteum, orbital fat protrudes above, and cheek fat descends below — the height difference across these three layers casts a shadow. Among Asian aesthetic patients in Toronto and Markham, tear troughs, dark circles, and under-eye bagsoverlap incidence exceeds 70%,This is why brightening eye creams alone can never fully address the problem.

Why is it not “dark circles”?

Dark circles are a pigmentation concern (pigmented type / vascular type); the tear trough isStructural hollowing。The two often coexist, but the treatment logic is entirely different — filling the hollow can simultaneously reduce the appearance of dark circles.

Why is it not “under-eye bags”?

Under-eye bags are caused by lower eyelid orbital fat protrusion; tear troughs are the hollow beneath that fat. Together they create an S-shaped contour — the convex bulge above and the concave groove below — making bags appear more prominent. Filling the tear trough actually reduces the visual impact of the bags.

Why is this particularly pronounced in Asian patients?

Asian patientsrelatively flat periocular bone support, thin subcutaneous fat, and tightly adherent orbital septum ligament attachments,The anatomical tear trough depression is typically deeper in Asian faces than in Caucasian patients — which is why a tired appearance often becomes noticeable after the age of 25.

Why must the supine tilt test be performed first?

Tilt head back 45° toward the ceiling: hollow disappears → yeslaxity / volume-loss type, may be suitable for filler; hollow does not disappear → Ligament-anchored type, requires precise deep supraperiosteal filler placement or ligament release.

Clinical note:Many clients have been told elsewhere that “a little filler will do the trick,” only to end up with Tyndall blue discoloration, puffiness resembling an eye bag, or a result that actually makes them look older — all classic outcomes when tear trough subtypes are not accurately assessed. Dr. Xu’s protocolThe first step is always classification + supine tilt test + transillumination assessment, rather than proceeding directly to injection.

CAUSES

The five main causes of tear troughs — it is not simply “aging”

Tear troughs can appear as early as one’s mid-twenties and are not purely a sign of aging. In Asian faces, tear troughs are typicallyAnatomical structure + volume loss + skin changesmultifactorial in origin.

1. Congenital nasojugal ligament attachment

Orbicularis Retaining Ligament(the orbicularis retaining ligament) anchors the skin tightly to the periosteum, creating a natural groove at the ligament attachment — this isCongenital structural tear troughat the root of the issue, independent of age.

2. mid-face volume loss

30 With age, descent of the malar fat pad combined with infraorbital bone resorption causes the tear trough to extend downward intoLid-cheek junctionandZygomatic hollow。This is the most common type among Asian women over 35.

3. Orbital septal fat herniation (pseudoherniation / false eye bags)

anterior prolapse of lower eyelid fat, causing the tear trough torelative hollowingmore pronounced. In these cases, filling the tear trough alone would make the under-eye bags appear more prominent — a combination approach is needed.

4. Skin thinning / pigmentation

The periocular skin is the thinnest on the body (approximately 0.5 mm) — thin enough to reveal the purple hue of the orbicularis oculi muscle and the underlying vasculature. Thisvisual dark circlescompounding the tear trough shadow and intensifying a tired appearance.

5. lifestyle factors / chronic inflammation

Sleep deprivation, allergic rhinitis, frequent eye rubbing, and prolonged screen time all worsen periocular pigmentation and accelerate collagen loss. Any injectable treatment should be paired with appropriate lifestyle management.

DIFFERENTIAL DIAGNOSIS

4 periocular conditions commonly confused with tear troughs — key differential diagnosis points

Misclassification leads to incorrect treatment. The table below summarizes the 4 most clinically common differential diagnoses that Dr. Xu encounters.

Vascular Dark Circles

NatureThin skin + visible vasculature
Head-tilt testColor has lightened but is still present
Triggering factorsThin skin + allergies / rhinitis

⚠️ Caution:Clinically, approximately 40% of patients who present requesting tear trough filler are reclassified after assessment as “primarily under-eye bags” or “primarily pigmentation.” Treating bags as hollows makes the puffiness worse; treating pigmentation as volume loss wastes money while the shadow remains — these are the most common frustrations among our Asian periocular patients.

ASIAN SKIN FOCUS

Why do Asian tear troughs require a dedicated Fitzpatrick III–V specialist protocol?

Blind spots in mainstream North American clinic practice

Most medical aesthetic clinics in North America base their tear trough filler protocols on Caucasian facial anatomy (Fitzpatrick I–II, high nasal bridge, deep orbital bones). Applying these protocols directly to Asian faces consistently creates three core problems:

  • ✗ Injection plane too superficial — causes Tyndall effect —— Asian periocular skin is thinner — superficial injection carries a high risk of Tyndall blue discoloration
  • ✗ over-injection causing visible swelling —— Asian orbital bones are flatter; doses calibrated for Caucasian anatomy can cause puffy rolled-eyelid swelling on Asian faces
  • ✗ Overlooking mid-face integration —— In Asian patients with a lower zygomatic arch, filling the tear trough alone does not address the underlying lack of structural support.

Our tear trough protocol is built fromKorean and Japanese oculoplastic specialty trainingframework, built on the skeletal support characteristics of Asian faces — not adapted from a North American Caucasian template.

TREATMENT OPTIONS

6 tear trough treatment options — selected by subtype

There is no one-size-fits-all tear trough filler. We combine the appropriate fillers and therapies based on the supine tilt test, mid-face assessment, and skin thickness.

recommended

Belotero Balance

CPM technology — monophasic HA with excellent spreadability, suitable forthe lid-cheek junction transition zone extending laterally from the tear trough —— Used in combination with Volbella across different zones to ensure a smooth, natural transition without visible edges.

Adjunct

skin booster injection (glutathione + polynucleotide)

Micro-droplet injections into the superficial dermis of the lower eyelid to thicken thin skin and reduce the visual blue translucency — used alongside filler for patients presenting with vascular dark circles in combination with tear trough hollowing.

Adjunct

hyaluronidase (Hylenex)

Used forRevision of failed cases from other clinics:Dissolves over-injected or incorrectly placed prior filler, allowing 8–12 weeks before redesigning treatment. Also serves as the first-line emergency response for Tyndall effect.

Adjunct

Juvederm Voluma(mid-face support)

deep supraperiosteal injection at the zygomatic arch,Restore mid-face support firstthen address the tear trough — this is a critical preliminary step for patients over 35 with volume loss, and is frequently overlooked.

Dr. Xu’s book systematically explores the aesthetic principles of Asian faces and the mid-face injection framework. Chapter 5 is dedicated entirely toAsian Periocular Anatomy (Tear Trough / Aegyo-sal / Dark Circles): Anatomical Differences and Minimally Invasive Aesthetic Solutions,is one of the most accessible Chinese-language specialist resources available to medical aesthetic clients across North America.

「”The Asian periocular area is not simply a smaller version of the Caucasian one. Our orbital bones are flatter, our skin is thinner, and our retaining ligaments attach more tightly — all of which means the injection plane, dosage, and instrumentation for tear trough treatment must be entirely redesigned.”

TORONTO PATIENTS ASK

Toronto Tear Trough Treatment FAQ

Answered personally by Dr. Dan Xu — drawn from the most frequently asked questions by tear trough clients in Toronto and Markham.
Are tear troughs and dark circles actually the same thing?

They are not the same concern, but they frequently coexist. The tear trough isStructural hollowing(groove), while dark circles arepigmented / vascular discoloration(become darker). When both are present, the shadow effect is amplified. The first step in Dr. Xu's protocol is to differentiate the primary cause using a head-tilt test and transillumination assessment — tear trough-dominant cases are treated with HA filler, pigmentation-dominant cases with low-energy PicoSure Pro + Cyspera, and vascular-dominant cases with skin booster injections to thicken the dermis combined with Vbeam®.

How long does tear trough filler last?

depending on filler choice and individual metabolism:Volbella Typically 12–15 months; Restylane Eyelight 12–14 months; Belotero (transition zone) 9–12 months。Clients with faster metabolism (highly active or younger individuals) may experience shorter duration of results. The periocular area metabolizes filler more slowly than the lips, so results generally last longer. We recommend an annual assessment — small top-up treatments produce more natural outcomes than a full re-fill each time.

Tyndall What causes the bluish band (Tyndall effect)? Can it be avoided?

Tyndall effect is HA Filler placed too superficiallywhen filler is placed superficially, light scattering through the skin produces a blue-grey discoloration — particularly common in the thin periocular skin of Asian patients. Prevention methods: (1) Deep supraperiosteal injectionrather than subcutaneously; (2) use low-cross-linked Volbella or Eyelight; (3) inject slowly with a 27G blunt cannula. Should it occur, it can be dissolved immediately with hyaluronidase (Hylenex) and typically resolves within 5 minutes.

Why did I look more swollen or older after having it filled elsewhere?

3 most common causes of treatment failure: (1) Skipping the mid-face assessment —— 35+ the client's zygomatic arch has already descended — filling the tear trough alone would flatten the mid-face and accelerate an aged appearance; (2) overdosage —— more than 0.5 mL per side can cause swelling on the flat orbital bone typical of Asian anatomy; (3) injection plane too superficial —— Subcutaneous injection can cause puffiness resembling a rolled eyelid. When Dr. Xu manages these revision cases from other clinics, the typical approach is to first dissolve the filler completely with Hylenex, then redesign the treatment plan 3 months later.

Should tear trough filler be injected with a blunt cannula or a sharp needle?

Dr. Xu's consistent first choice for Asian periocular treatment 25G–27G Blunt cannula。A blunt cannula does not cut through blood vessels, significantly reducing the risk of vascular occlusion and bruising, and makes it much easier to maintain the injection plane at the supraperiosteal level. Sharp needles are used only at specific entry points, such as the lateral zygomatic arch access channel. Some clinics in North America routinely use sharp needles for multiple subcutaneous injection points — on the thin skin typical of Asian patients, this carries a high risk of Tyndall effect.

Will there be swelling after treatment? How soon can I resume normal social activities?

Swelling from deep supraperiosteal injection with a blunt cannula is typically minimal. Most patients are socially presentable within 24 hours,Any bruising can be covered with makeup and typically resolves within 3–5 days. Recommendations: (1) apply ice on the day of treatment; (2) avoid inverted yoga poses, strenuous effort, and saunas for 24 hours; (3) no vigorous exercise for 48 hours; (4) avoid alcohol for 1 week. For important events, we recommend completing your injections at least 2 weeks in advance.

I have under-eye bags — can I still have tear trough filler at the same time?

The degree of under-eye bags must be assessed first. Mild fat protrusion mayCombination approach:Filling the tear trough reduces the visual prominence of eye bags and creates a smoother transition. For moderate-to-severe true eye bags (with significant fat pad protrusion),surgical lower eyelid fat repositioning is recommended for prior assessment,Filling the tear trough alone can make true eye bags appear more prominent. During consultation, Dr. Xu performs palpation and a supine-position assessment — if surgical intervention is indicated, you will be told candidly.

Can tear trough filler be done during pregnancy or while breastfeeding?

No.Although hyaluronic acid is naturally absorbed by the body, there is insufficient clinical safety data for use during pregnancy or breastfeeding. Hormonal fluctuations during these periods also increase the risk of bruising, swelling, and vascular reactions. We recommendPost-partum + after complete weaningbefore beginning an assessment. During this period, if visual fatigue is a concern, focus on improving sleep, consistent sun protection, and using an eye cream containing vitamin K or caffeine.

Can I apply makeup or exercise right after the treatment?

24 hours post-procedure, light makeup is permitted (avoid entry point sites); full makeup after 48 hours; avoid eye-area exfoliation, acids, or chemical peels for 1 week. Regarding exercise: no strenuous activity for 24 hours, light activity may resume after 48 hours, and normal exercise after 1 week. After treatment, avoid pressing firmly on the eye area and do not sleep face-down(to prevent filler migration).

I live in downtown Toronto / North York / Richmond Hill / Scarborough — is it worth making the trip to Markham?

ID Cosmetic Clinic Located at 30 Gibson Dr., Markham — approximately 30–40 minutes by car from downtown Toronto, 20–25 minutes from North York, 15–20 minutes from Richmond Hill / Vaughan, and 20–30 minutes from Scarborough. Conveniently situated near the Highway 7 + 404 / 407 interchange. The tear trough is the thinnest and most sensitive injection zone on the face,The window to manage Tyndall effect or vascular occlusion is very narrow. Choosing a clinic where a physician personally evaluates and performs the treatment matters far more than convenience of location. The cost of correcting a poorly placed filler is far greater than a single commute.
